AB0443 BIOMARKER SUCH AS IL-17, IL-21 AND TIMP-1, IS USEFUL FOR PREDICTING THE PATHOPHYSIOLOGY OF CONNECTIVE TISSUE DISEASE-ASSOCIATED PULMONARY HYPERTENSION

Background: Connective tissue disease-associated pulmonary hypertension (CTD-PH) is constructed by a variety of pathologies, including cardiac, pulmonary, and vascular involvement, as well immune abnormalities. Because its various constructs, it difficult for many respiratory physicians, cardiologists, rheumatologists to determine treatment strategy CTD-PH. In addition, CTD-PH has different pathologies from iPAH, there are cases in which immunosuppressive therapy effective. These suggests that the two PAHs may have pathogenesis, inflammation artery. However, not enough biomarkers distinguish pathologies. On other hand, been reported cytokines such TIMP-1, Interleukin (IL)-6, IL-17, IL-21 involved pathogenesis or vasculitis. (1,2) relationship between these cytokine expression fully clarified. Objectives: To clarify profile clinical features, change hemodynamics treatment, association with effectiveness therapy. Methods: Patients suspected PH was included. At time cardiac catheterization(RHC), sera pre post-capillary were collected MCP-1, IL-17 IL-21, IL-12p70 IL-6 analyzed ELISA(ABCAM UK, Ella simple plex USA). The following data collected: age, gender, underlying disease, complication interstitial lung (immunosuppressant vasodilator), hemodynamics. Furthermore, we investigated data. Results: 15 CTD-PH, 13 non-CTD-PH, 6 non-PH analyzed. (SSc 12 cases, MCTD 7cases, SLE 2 others cases) 28 diagnosed RHC. There positive correlation mean arterial pressure all case. IL-6, TIMP-1 tend be high SSc-PH cases. Non-SSc-PH, high. who improved decreased. Conclusion: Biomarker profiles capillaries differ depending on disease. suggested therapeutic effect.
